He is survived by: his spouses, Kirsten Glantz and Samantha Hawksworth; his children, David "Bubba" Hawksworth (Natalie), Austin Hawksworth (Caeley) and Markee Hawksworth; his granddaughter Bella Hawksworth; his mother Rosalie Hawksworth; and also, Jessica Bertolas (James), His, Alan Anderson (Sandy), Celeste Anderson (Tom Beeler), Arianna Bertolas, Nick (Jordyn), Ryan Johanns, Colton Kerrigan (Jennifer) and Adam Mahne.
In lieu of flowers, contributions can be made to the National Kidney Foundation in David's name.
