He is survived by: his wife Annette Mattox; and his cousins, Susan Warren (Bill), Tyler Smith (Janice), Connie Atwood (Curt), Mike Okerberg (Brenda), Jim Okerberg (Jennifer) and Jim Campbell (Debbie). He is also survived by his extended family included numerous aunts, uncles and cousins.
