He is survived by: his daughter Amber Allen-Sladick; his grandchildren, Hailey Sladick, Orion Sladick and Jacob Sladick; his sisters, Teresa Green and Candy Allen; and his niece Sheree May. He is also survived by great nieces, nephews, as well as a great lifelong friends.
