He is survived by: his parents, Staffany Diane Moore and Ananias Dimitrius Moore.
In lieu of flowers, the family is requesting stuffed animals. The animals will be donated to Rocky Mountain Children's Health Foundation, 5394 Marshall Street, Suite 400, Arvada, CO 80002, in Uriah's memory.
