He was predeceased by: his parents, George Sr. Petak and Regina Petak; and his brothers, Frank and David. He is survived by: his wife Nancy Farrell (Farrell); his son Brian (Heidi); his grandsons, Josiah, Jonathan, Joshua and Justin; his son Todd (Shannon); his grandchildren, Bethany and MaKaylee; his child Meigan Rivera (Armando); his grandson Diego; his great grandchildren, Alayna and Rylan; and his siblings, Dan (Debbie) and Teri Scofield (Brad). He is also survived by many nieces and nephews.
