He was predeceased by: his parents, Harry Herbert and Gladys Herbert; his children, Donald Herbert and Sandi DeBruler; and his brother Bobby Herbert.
He is survived by: his children, Scott Herbert (Jessica) of Castle Rock, CO, Eddie Herbert of Socorro, NM and Susan Dixon (Doug) of New Paris, OH; his sister Shirley Evans of Connersville, IN; and his grandchildren, Jacob, Kaitlyn Herbert, Brandie Burden, Jessica Miller (Chris), Hannah Dixon and Nathan DeBruler. He is also survived by 4 great-grandchildren and many nieces and nephews.
