He was predeceased by: his parents, Bill Flaherty and Jean Flaherty (Daniels); his nephew Michael Flaherty; and his niece Lorie Flaherty Kimble. He is survived by: his wife Michelle; his daughters, Jordan, Hannah and Emily; his brother David Flaherty (Barbara); his stepmother Barbara Flaherty; his great-niece Alexis; and his cousins, Kirsten Laird (Warren), Josh Sundberg, Sarah (Miles) and Kensey (Joshua). He is also survived by his family and friends.

In lieu of flowers, the family requests donations be made to the American Heart Association in Patrick's honor.
