He was predeceased by: his parents, Glenn Howard Cook and Martha Lyzette Cook (Anderson); and his wife Eloise Opal Cook (Baker). He is survived by: his children, Sondra Jean Janssen (Cook), Jeffrey Wayne Cook and Charles Ellis Cook. He is also survived by his three children, ten grandchildren and eleven great-grandchildren.
