He was predeceased by: his brothers, Robert, Herbert Jr. and Homer Frank. He is survived by: his wife Jeanette; his children, Philip Headrick, Caroline Cummins, Laura Evans and Ellen Headrick; his grandchildren, Scott Headrick, Kyle Headrick, Jessica Cummins and Joseph DeYoung; and also, Homer Frank and Lewis.
In lieu of flowers, Lewis' family kindly requests memorial donations may be made to First United Methodist Church of Boulder, Memorial Sloan Kettering Cancer Center, the International Essential Tremor Foundation or Kansas State University Foundation.
