He was predeceased by: his parents, Edward Holm and Barbara Holm (Tormey); his daughters, Crystal and Carolyn Holm; and his siblings, Steven, Shorty, Rick, Anna and Martha.
He is survived by: his wife Cheril Holm; and his children, David Jr. Holm (Brittany), Mary Maynard (Kevin) and Dean Holm (Jackie). He is also survived by 13 grandchildren, 1 great grandchild and 11 siblings.
