He is survived by: his parents, Jeremy Shook and April Shook; his uncle Jason; his cousins, Roy, Ben, Jonathan and Wesley; his sisters, Janessa, Josie and Julia Hart (Mark); his nieces and nephews, Eli, Maya and Adelyne; his sister Jillian Lebsock (Davis); his daughter Leona; and his grandmothers, Mary Ann Shook and Mary Jane Rozell. He is also survived by his aunts, uncles, cousins, extended family, and the many friends.

In lieu of flowers, the family requests that donations be made to the Akron Volunteer Fire Department where Jared passionately served his community.
