She was loved and cherished by many people including: her parents, Morris Stark and Dorothy Stark; her husband Lee Forman; her daughters, Monique Alexander (Phil Alexander) and Candi Whitsel (Bob Whitsel); her grandchildren, Jacob Alexander (Hannah Alexander), Zachary Alexander (Savannah Ralli) and Dakota Whitsel; her great grandchildren, Ellie Alexander and Ezra Alexander; and her sister Illene Gross. She was also cherished by many nieces and nephews.
In lieu of flowers, the family asks that donations be made in Sheila's name to Suncrest Hospice or St. Jude Children's Research Hospital.
