She is survived by: her husband Keith McIntosh; her daughter Kimberly Blaylock (Matt); her granddaughters, Brooke Smith (Zach) and Caitlin; her great grandchildren, Elle and Theo; her siblings, Alfred (Pat), Frank (Dolores), George (Rosemary) and Joan Rey; and her caregiver Georgia. She is also survived by numerous nieces, nephews, and cousins.
