He was predeceased by: his parents, William Lewis and Ethel Lewis; his sisters, Carol Fischer and Ann Lewis; and his sister-in-law Andra Lewis.

He is survived by: his wife Amy; his daughters, Adriana Chiu (Cameron Chiu) and Angelina Lewis; his brothers, Dan Lewis and Steve Lewis; and his siblings-in-law, Tammy Lewis and Rob Fischer. He is also survived by several nieces and nephews, and many special friends.
