He is survived by: his wife Cheryl of Napa; his children, Cori (Wayne) of Napa, John (Kay) of Sacramento, Jeremy (Noemi) of Napa, LaDean Fort (Brian) and Terry of San Francisco; his grandchildren, Daniel, Kari, Kaitland, Jay Jr., Shawn, Trent, Jayden, Maya, Camila, Taji and Zanya; his great grandchild Dean; and his brothers, Doug and Dennis.
