He was predeceased by: his sister Maxine. He is survived by: his children, James, Sean, Craig and Portia; and his wife LaDonna.
The family request that in lieu of flowers, please make donations to San Carlos Cathedral in Monterey; Jack's longtime parish and where he served as a Eucharistic minister for many years.
