He is survived by: his friends, Santa Cruz, Rock, Roll and Blues; his wife Debbie Cushman; his son Jack Jr.; his grandchildren, Danicka Jean, Jack Maverick and Riley Iris Cushman; his brother Bob Andruss of Washington; and his close friends, Billy and Dennis. He is also survived by many nieces and nephews.
