He was predeceased by: his parents, Arthur I Lewis and Dorothy Lewis (Mooneyham); and his siblings, Arthur, Daniel, Dorothy and Michelle.
He is survived by: his wife Annie Saavedra Lewis; his sons, David Jr. and Anthony Lewis; his grandchildren, Allize (Amir), Alyssa, Derek, Alana and Gavin; his sisters, Cynthia and Catalina; and also Joe Silva. He is also survived by nieces and nephews, cousins, five godchildren and friends.
