He is survived by: his sister Rose Perkins; and his children, Jeanine Oien, Jeff, Lisa Oien and Elissa Rush. He is also survived by ten grandchildren and ten great grandchildren.

In lieu of flowers, Jack made the request that donations be sent to Pastors to Prisoners.
