He is survived by: his brother Larry Kozlowski (Nancy); his children, Wayne Kozlowski (Kristine), Lisa Kozlowski and Kristin Kozlowski; and his grandchildren, Michael Kozlowski, Cory Kozlowski, Brandon Helvey and Tory Helvey.
