She is survived by: her husband Jack Churchill of Hermosa; her sister Charlotte Carter of Spearfish; her children, Jackie Workman (Wane) of Roosevelt, UT and Jim (Janice) of Goodyear, AZ; her grandsons, Joshua Haakinson (Kim) of Roosevelt, UT and Jeremy Haakinson (Jamie) of Vernal, UT; and her great grandchildren, Lauren Haakinson, Lexie Haakinson, Abigail Haakinson and Emma Haakinson. She is also survived by numerous nieces and nephews.
