He was predeceased by: his parents, Coy Hazel and Pearl Hazel of Sikeston, Missouri.; and his siblings, Coy Jean and Patricia De Leon. He is survived by: his former spouse Sandy Cox; his daughter Holly; his wife Carol Lynn Kelley; his daughters, Teri Nichols (Mason) and Holley McGowan (Tim); his sister Donna Larsen (Lee); his stepchildren, Kelly Crane and Kristin Baker; his grandchildren, Hyde Nichols, Kate Poloni (Kyle), Michael Nichols (Cailyn), Robert, Nicholas, Hope McGowan, Kathryn, Ryan Ward and Hannah Tebo; and also Carol.
