He is survived by: his wife Julie; his children, Tyler (Sarah), Alyssa and Makayla; his parents, Iris Middleton and Richard James Middleton; and his siblings, Diane Simeone, Karen Fields and Brian Middleton. He is also survived by numerous nieces, nephews, and cousins.

In lieu of flowers, donations can be made towards a fund for Makayla to continue her softball career and for her college education.
