He was loved and cherished by many people including: his spouse Gail K. Kubasak; his children, Tammy and Shawn; his grandchildren, Lauren, Caitlin, Kelly and Jake; his great grandchildren, Kayden, Landon and Lily; and his caregiver Michelle Foster.

In lieu of flowers, the family kindly requests donations to be made to St. Judes Hospital in Jerry's name.
