He was predeceased by: his parents, Jay H Polen and Virginia K Polen. He is survived by: his siblings, Martha DeBenedetti (Marti) of Kailua Kona, HI and Matthew Polen (Sandy) of Ewa Beach, HI; his nieces, Angelica Olmstead (Tyler) of Denver, CO, Gabriella Fratta (Louis) of Denver, CO and Veronica Frye (Kellen) of Pittsburgh, PA; and his great-nephew Archer Fratta.
