He was predeceased by: his parents, Phillip Rafeal Lewis and Pasquelita Evans; and his son Bodie Phillip Lewis. He is survived by: his wife Vera Lewis of Winslow; and his children, Ken Lewis of Holbrook, Roland Lewis of Holbrook and Kira Lewis of Winslow. He is also survived by 4 grandchildren, and 3 great grandchildren.
