She is survived by: her husband Jerry Mack Forde; her sons, Timothy Forde (Susan) and Daniel Forde (Kimberly); her grandsons, Carson Forde and Cameron Forde; her daughter Collette Root (Richard); her granddaughters, Katrina Baldwin (James) and Courtney Root (Lynn); her daughter Felicia Slaton (Chad); her granddaughter Lacie Forde; and her siblings, Ellen Koan, Lottie Lindall, Bonnie Lewis-Noble, Ben Lewis, Bill Lewis, Ed Lewis and Adam Lewis.
