He is survived by: his parents, Linda Hendrickson and Lee Hendrickson (Judy Hendrickson); his daughters, Scarlett, Madison, Brielle and Brooklyn; his grandfather George Hendrickson; and his siblings, Erick Hendrickson, David Hendrickson (Stephanie), Seth Hendrickson, Melanie Anderson and Jennifer Maughan (Ryan). He is also survived by many family members and friends.

In Lieu of flowers, a GoFundMe has been established to support Sam's girls during this difficult time.
