He is survived by: his parents, Allan L. Church and Janet F. Church (Oldfield) of Sierra Vista, AZ; his sister Shara (John Forrister) of Prosser, WA; his nieces, Lilah Forrister and Rachel Forrister; and his uncles, Randy Oldfield, Timothy Oldfield (Catherine) and Elmer Dale Church (Laura).
