He was predeceased by: his grandparents, Billy Hackworth of Jenks, OK, Boyd Farar of Yuma, Joanne Farar of Yuma, John Henry Cook of Yuma and Frances Cook of Yuma; his parents-in-law, Paul Methvin and Patricia Methvin; and also Marion Scarborough.
He is survived by: his wife Mary Dawn Cook of Show Low; his daughters, Triniti Danielle Cook of Show Low, Triciamae Darlene Cook of Show Low and Tayler Dawn Brown (Davis) of Tempe; his parents, Hollie Jo Cunningham (Ernest) of Snowflake and Henry Thomas Cook (Pamela) of Mounds, OK; his siblings, Thomas Dean Cook (Bekki) of Yuma and Wendi Loree Nelson (Spike) of Pinetop; and his brother-in-law Paul Methvin of Litchfield Park. He is also survived by several aunts, uncles, cousins, neices, nephews and great friends.
