He was predeceased by: his parents, Cecelia Leroy (Johnson) and Leroy J. Polk Sr. Leroy. He is survived by: his sons, William H. Polk of Sells, Joseph R. Polk of San Carlos, Ricardo L. Polk of Mendota and Isaiah Polk of San Carlos; and his siblings, Lupita James of San Carlos, Consuela Gooday of San Carlos, Stefanie Dillon of San Carlos, Ramon Polk of Mesa and Chris McIntosh of San Carlos. He is also survived by five grandchildren.
