She was predeceased by: her son Donald Slavin; and her parents, John Kolak and Theresa Kolak. She is survived by: her sons, Jeff Slavin (Vikki) of Dacula, GA and Richard Slavin of Phoenix, AZ; her grandsons, Lorenzo, Jakob, Andrew, Mason, Tanner and Logan; and her siblings, Janice Kolak, Mary Ann Goad (Gary), Diane Helander (Denny) and John Kolak Jr. (Marilynn). She is also survived by many nieces and nephews.
