He was predeceased by: his parents, Jean Mary Geppert and Salvatore Michael Formicola, Sr.; and his son Mackenzie Raymond Formicola. He is survived by: his wife Susie Formicola; his children, Parker and Maree Formicola; his grandchildren, Graysen, Emric and Paisley; and his sisters, Mary Jean Jones (Formicola) and Sharon Joanne Formicola.
