She is survived by: her parents, Jesse Nelson Perkins and Mable Stratton Perkins; her sons, Ray Addington (Fayrene) and Jay Addington (Sandy); and her siblings, Norma Arnett and Roy Perkins. She is also survived by numerous grandchildren and great grandchildren.

In lieu of flowers, donations to Hospice of the Valley would be greatly appreciated.
