He was predeceased by: his parents, Barbara Jean Olson and Elmer Madison Jaques; his former wife Marilyn Lee Pratt; and his sister Sharen. He is survived by: his children, David, Clain, Andrea and Matthew; his brother Gary (Renee); his nieces and nephews, David, Clain (Michele), Andrea and Matthew (Camille); his grandchildren, Charlie, Chandler, Kristin, Madison, Michael and Cooper; his great grandchildren, Emery and Maggie; and his stepsons, Robert and Larry (Teri).
In lieu of flowers, donations may be made to the LDS Church Missionary Fund or St. Jude's Children's Research Hospital.
