He was predeceased by: his parents, Alvis Cook and Birdie Cook; and his siblings, Tom and Rita Carol. He is survived by: his wife Judy; his brothers, Mark (Sharon) and Brent (Becky); and his sister-in-law Jan. He is also survived by nine grandchildren, four great-grandchildren.
