Smith; her children, Micah Forni (Jason) of Mesa, AZ and Ben Smith of New Haven, CT; her grandchildren, Alina, Ethan and Maeze Forni; her stepfather Harry McClintock of Heber Springs, AR; her sisters, Kelli Grate (Shannon) of Wichita, KS, Jennifer Bowles (Jace) of Wichita, KS and Jessica Sebok (John) of Kernersville, NC; and her stepsiblings, Terry Lee Willis (Stan) of Tulsa, OK and Mike Garrett (Tina) of Wichita, KS..

A celebration of life was held on Tuesday, July 16th 2024 at 6:30 PM and on Saturday, September 21st 2024 at 9:00 AM at the Gold Canyon Community Church (5810 S Alameda Rd, Gold Canyon, AZ 85118).
