He is survived by: his wife Shelly Salazar; his mother Helen Salazar; and his children, Alissa Velasquez (Robert), Marques Perez (Danielle), Nichole Steveson (Jason), Jessica Gatewood (Andrew), Candace Van Haren and Brady Van Haren.
