She is survived by: her husband Chad; her children, Colter, Kira, Evelyn and Wade; her parents, Freddie Chism and Cheryl Chism; her brothers, Freddie Jr. Chism (Darice), John Chism (Jessica), Justin Chism, Dustin Hulett (Melissa) and Jacob Chism (Taylor); her grandparents, John Chism (Linda) of Globe, Virginia Baer of Globe, AZ and Nancy Johnson of Rio Rancho, NM.; and her child Beau Hulbert (Jericho).
