He was predeceased by: his parents, George and Addie May; and his brother Robert "Bob". He is survived by: his daughter Kirsten (Brandon); his grandchildren, Michael (Tristen), Thomas, Allora and Madalyn; his great grandchild Jude; and his former wife Debbie. He is also survived by his family, friends.
In lieu of flowers, the family kindly requests that contributions be made to the American Society for Suicide Prevention, a cause that Dave held dear and championed throughout his life.
