He was predeceased by: his parents, William R. Eubanks and Dwight. He is survived by: his wife Andrea Anderson Eubanks; his children, Shawn Eubanks (Tammy) and Danette Otzen; his grandchildren, Brandon Eubanks (Gina), Christian Good (Ryan), Jacob Otzen and Logan Duncan; his great grandchildren, Powell Eubanks and Bishop Eubanks; his siblings, Jerry Eubanks (Belinda), Cheryl Adams and Linda Piper (Steve); his grandparents, Homer Joy, Edna Joy, George Daves and Mable Daves; and his nephews, Tony Adams and Floyd Adams.
A memorial Service and Celebration will be Sat, Nov 17th @ 10:00am-1:30pm at the Elks Lodge, 2101 N. San Francisco St. Flagstaff, Az 86001 In lieu of flowers, please make donations to Northland Hospice at 452 N. Switzer Canyon Drive.
