He was predeceased by: his parents, Jesse Collinsworth and Norma Collinsworth; and his sister Karen. He is survived by: his former wife Lisa; his stepgrandchildren, Hannah, David and Daniel; his wife Donna; his children, Jason, David (Laura), Daniel and Hannah Chapman (Adam); his stepdaughters, Kristi Manzer (David), Tiffany Hulsebosch (Josh) and Lindsey Amaro (Izzy); his siblings, Jim (Sue), Ron (Paula), Nancy Diepstraten (Ted) and Gloria Mason (Bill); and his uncle Henry Warren. He is also survived by many cousins, numerous nieces, nephews, great nieces and nephews, and great-great nephews.
