He is survived by: his daughter Jamie Anne Forte; his wife Laurie; his son-in-law David Hammond; his brother Dr. Robert Forte (Patty); and his nieces and nephews, Colleen George, Robert Forte, Brian Forte, Jason Rexford, Maddie and Jared Monchnik.
