He is survived by: his parents, Dallas Middlekauff and Elizabeth Middlekauff; his spouse Alice Maybury Middlekauff; his sister Joy Artus of Concord, NC; and his children, Steve Middlekauff (Laura) of Conway, Susan Johnston (Donnie) of Wynne and Nancy Thompson (Todd) of Miramar Beach, FL. He is also survived by eight grandchildren; and three great-grandchildren.
In lieu of flowers, memorials may be sent to Village United Methodist Church, 200 Carmona Road in Hot Springs Village, AR 71909.
