He was predeceased by: his parents, Clero Mullins and Alda Stepp Mullins; his son David F. Mullins M. D.; and his siblings, Herla Mullins, Dew Yates and Wilma Tate.

He is survived by: his wife Wanda Mullins; his daughters, Deborah Mullins Watt and Sheila Heng (Dave); and his brother Loren Mullins (Wanda). He is also survived by six grandchildren, five great grandchildren.
