He was predeceased by: his father Henry Talley Cook; and his siblings, Henry Jr., Osmond, Harold, John, Richard, Laverne Cook, Fannie Patterson, Doris Stockman, Marlene Huntley and Mary Roush.
He is survived by: his wife Betty Merlene McGuire Cook; his sister Marion Patricia Williams of Lawrenceville; his children, Rebecca Maples (David) of Ward and James Cook of Searcy; and his grandchildren, Trinity Bowers (Sam) of Lonoke, Jestiny Gibson (Jeffery) of Lonoke, Blaine Maples (Haley) of Quitman, Shyanne Stephens (Josh) of Hibbing and Keleah Gray (Talon) of Conway. He is also survived by 15 great-grandchildren and many nieces, nephews.
