He is survived by: his father Harvey Ballard; his wife Teresa Ballard; his sons, Daniel Lynn Ballard (Christina) and Noah Wayne Ballard; his grandchildren, Khloe Ballard and Samuel Ballard; and his sisters, Beverly Thomason (Lynn) of Deborah DeMent, C. J, Bastian (Chuck) and Trish Lorenzen (Rick). He is also survived by many other relatives and friends.
