He was predeceased by: his father Freddie Allen Lewis, Sr.. He is survived by: his children, Sara Lewis, Michelle Blake (Clint), Chelsea Lash (Chris) and Ashtyn Lewis; his girlfriend Amy Caldwell; his mother Peggy Lewis; and his brother Freddie Lewis. He is also survived by three grandchildren.
