He was predeceased by: his parents, Ann Forte and Patrick Forte of Sheridan; his sisters, Laura Ann Reddin and Melissa Lee Caster; and his uncle Michael Dan Forte.
He is survived by: his aunts and uncles, Matthew Forte (Janis), Charles Thomason, Bekky Forte and Linda Gene Solano of Pine Bluff. He is also survived by cousin, caregiver.
