He is survived by: his wife Diane Wormington Smith of Sheridan; his children, Mark Allen Smith of Sheridan and Amanda Smith of Sheridan; his siblings, Jerry Smith of Sheridan, Terry Smith (Deanna) of Sherwood and Patricia Smith of Sheridan; and his granddaughter Kayla Haynes. He is also survived by extended family members and friends.
